COH Rehab aims to keep website information clear and current, but information may change and may not address every person's circumstances. Content is provided for general information about COH Rehab's services and referral pathways.
Do not rely on this website for urgent clinical decisions. In an emergency, call 000 or attend the nearest emergency department. For personal medical advice, contact an appropriate treating practitioner.
3. Referrals and service availability
Submitting a form does not guarantee acceptance, availability, funding approval or commencement of services. COH Rehab may need to confirm consent, scope, clinical suitability, service capacity, location, required documents and insurer or funder approval before accepting a referral.
A clinical or service relationship begins only when COH Rehab confirms acceptance and any required arrangements. Referrers must provide information that is accurate to the best of their knowledge and must have appropriate authority to provide personal and health information.
COH Rehab's core focus is CTP rehabilitation. COH Rehab does not provide general workers compensation workplace rehabilitation. WorkCover-related scope is limited to services COH Rehab is authorised and available to provide, including relevant ADL assessment and OTT005 matters.
4. Intellectual property
Unless otherwise stated, website content and branding are owned by or licensed to COH Rehab. You may view or print content for personal, clinical or referral-related use. You must not reproduce, modify, republish or commercially exploit the content without permission.
5. Privacy and online forms
Personal information is handled as described in our Privacy Policy. Referral and consent forms are hosted by Jotform, whose own terms and privacy practices also apply when you use its platform. Do not submit information unless you are authorised to do so.

This website may link to third-party websites, forms and government resources. Links are provided for convenience and do not amount to control or endorsement of every statement, product or practice on those services. COH Rehab is not responsible for third-party availability or content.
