A clear view of function in real life
An Activities of Daily Living assessment looks beyond diagnosis to understand how a person is managing the tasks and roles that make up an ordinary day. The assessment considers current function, the home environment, personal goals, available supports and potential risks.
Our occupational therapists gather information through interview, observation and functional assessment, then provide a clear report addressing the referral questions.
Areas we may assess
- Personal care, dressing and grooming
- Meal preparation, cleaning and household tasks
- Mobility, transfers and access within the home
- Shopping, transport and community participation
- Fatigue, pain, cognition and task tolerance
- Equipment, home modification or support needs
CTP and WorkCover referrals
COH Rehab can complete ADL assessments for both CTP and WorkCover matters. For clarity, our WorkCover involvement is limited to occupational therapy assessment services; we do not offer workers compensation workplace rehabilitation or return-to-work provider services.
